Resource Coordination
Comprehensive Resource Coordination assists individuals in gaining access to medical, social, educational,
residential and vocational services and support needed to assist them in
living full lives in the community of their choice. Based upon the
preferences and needs of the individual and their significant others, this
support provides assistance with coordination and monitoring of services
available through the public system and the identification, development and
assistance with access to community and natural resources.
Core Objectives
- Person-centered - Through partnership with the individual and their
significant others the resource coordinator will assess needs and develop a
plan based on the preferences of the individual.
- Resource identification, development, coordination
and monitoring -
Resources include specialized services available through the public service
system for specific population groups, generic services and support
available to the general public, and natural support available to the
particular individual.
- Community education - Through assisting individuals with disabilities in
becoming active participants in their communities, the resource coordinator
educates the community about the needs and strengths of its citizens with
disabilities.
- Advocacy - Systems change, organizational collaboration, community
connecting and development, strengthening of naturally supportive
relationships.
Resource Coordination is primarily provided through two service models, Case
Management and Community Support Services.
